To send a referral, please fax a Patient Face Sheet and the Last Clinical Note to (501) 439-8135.

If you don't have a fax machine please call the clinic and we'll provide you with a secure, non-fax, HIPAA compliant option.

You can also share this website with your patient and have them follow our New Patient process versus the traditional provider referral route.

Our team will process referrals daily and reach out to the patient for scheduling within a few business days.
